Countermeasures 2: PDCA

The PDSA process (plan, do, study, act) evolved from a process developed by W. Edwards Deming who is considered the father of modern quality control. The Japanese in 1950 changed it to PDCA (plan, do, check act). We have modified the steps to plan, do, check, adjust. This change allows for adjustments that need to be made to fine tune the solution. This simple four step process, applied to any problem or deficiency such as how to raise HCAHPS scores, will allow the user to develop a solution that will solve the root cause of the problem.

The four steps are:

PLAN
Set the objective for the process. Determine if the is data available. If the is no data, what do we need to do to collect data? Establish a baseline. Set a measurable future state goal. From this, can we determine the possible causes of the problem? What LEAN tools can we use to determine the root cause of the problem?

DO
Develop and implement the solution. Collect data on the changes as a result of the implementation of the solution..

CHECK
Measure the results. What changed? How do the results compare with the expected results? What else needs to change?

ADJUST
Make the necessary changes. Implement the solution and ensure its hard wired. Ensure everyone is aware of the process changes. Document using a standard work sheet. Put the sheet on your gemba board and review regularly.

If you have not achieved your future state goal, start the process over from the beginning. Set real cycles of PDCA may be necessary to achieve the results you want. You may discover that you are doing the cycle to resolve an issue within the cycle.
